An Iraqi cameraman working for Reuters was killed during fierce clashes between US forces and rebels on Monday. Dhia Najim, 55, was covering fighting near his house in Ramadi when he was shot in the head. His last footage showed flashes from gunfire and smoke from explosions as US Humvees raced across a junction.

US Marines are gearing up for an expected offensive against insurgents and Islamist militants as part of efforts to pacify the country before January’s elections.
